This form seems to be payment information (and details of both parties involved in said policy) regarding payment of premium (or something related to insurance). The phrase "USO DO BANCO" is a field box that happens to not have any data entered, so I'm lost on translation with this phrase. Any thoughts on this would be appreciated.
