Texto de origem - inglês Common causes of TBI are motor vehicle crashes, falls, assaults or blast injuries. The injuries are categorized according to severity as mild, moderate or severe. In most injuries, damage occurs immediately when forces are exerted on the brain. Forces are exerted either directly, as in the case of penetrating or blunt impact, or indirectly as when the brain is subjected to strong forces of movement. There is a rich network of neural connections linking the brain for rapid transmission and coordinated functions. Injury to the connections can interfere with the brain’s ability to function efficiently.
Sometimes bleeding occurs, which results in additional damage to the surrounding brain tissue. In most injuries, the brain responds by swelling. Since the brain is enclosed by the rigid skull, swelling in one area can result in pressure and additional damage to the surrounding areas.
Skull fractures, or breaks in the bone surrounding the brain, may occur with or without damage to the underlying brain. The bone itself will heal over time. Additional treatment may be required if there is damage to the brain beneath the fracture.
Concussion is the mildest form of brain injury. When a person has a concussion he or she may pass out or lose consciousness for a short time, or experience memory loss or loss of other brain functions briefly. Treatment is usually not necessary, however, it is important to observe the patient closely for signs that a more severe injury has occurred.
In a contusion, a part of the brain is bruised and develops small areas of bleeding and swelling that can be seen on tests. Patients with contusions often complain of headaches, nausea and slow thinking. Patients with contusions must be watched closely for signs of additional swelling, bleeding or further injury of the brain.
Sometimes opposite sides of the brain are injured when the brain first strikes one side of the skull during the impact and then bounces back against the opposite side of the skull. This is called a coup contrecoup injury (Figure 3).
A generalized injury to the brain can occur when stretching or shearing forces are applied to the entire brain. This can result in damage to the connections, or axons, deep in the brain and is called a diffuse axonal injury. In response, swelling can occur throughout the brain. Since the axons cannot be repaired through medical intervention, treatment is directed at preventing further damage. Bleeding in or around the brain is very damaging and is often treated with surgery.
Bleeding that occurs between the skull and the dura, the thin covering over the brain, is called an epidural hematoma (Figure 4). If the blood clot is large or increasing in size it is treated with surgery to remove the clot and take the pressure off the brain.
Bleeding also can occur between the dura and the brain. The blood clot forms what is called a subdural hematoma (Figure 5). Acute subdural hematomas occur at the time of the injury and are often associated with damage to the underlying brain and severe swelling.
Chronic subdural hematomas occur most commonly in the elderly after a relatively mild head injury. In some cases, a small amount of blood in the subdural space will cause the space to enlarge slowly over time, producing increasing pressure on the brain. In both cases, surgery is often required.
An intracerebral hemorrhage is a blood clot within the brain (Figure 6). Surgery may be required to remove the clot, but surgery has significant risks because it may be necessary to go through areas of normal brain to get to the clot and remove it.
Occasionally, bleeding occurs into the spaces filled with CSF. The blood can block the flow of CSF so that pressure builds up in the ventricles, and the ventricles enlarge. This condition is called hydrocephalus (Figure 7). Hydrocephalus is often treated with a drainage tube, or ventriculostomy, that drains the fluid to a collection bag outside the body or under the skin into the abdomen.
Any injury to the brain can affect the intracranial pressure (ICP), the pressure inside the skull. Since the skull is rigid and cannot stretch, swelling, blood clots or blocked CSF can cause everything inside the skull to become tight and the pressure inside the brain to increase. Much of the treatment of traumatic brain injuries is directed at keeping the intracranial pressure as close to normal as possible.
Cerebral perfusion pressure is the pressure that pushes blood into the brain and supplies the brain with oxygen. If the blood pressure is low or the intracranial pressure is high, or both, then the cerebral perfusion pressure will be low, and the blood flow to the brain will decrease. This can cause a low oxygen level and possible damage to the brain (Figure 8).
Evaluating a TBI Patients with traumatic brain injuries often undergo extensive testing to determine the extent and location of the damage.
A health care provider performs a physical exam specifically including a complete neurologic exam which assesses functions of the brain such as:
• strength
• sensation
• speech
• vision
• level of consciousness.
X-rays, which show only bony structures, are used to look for fractured bones. CT (computerized tomography), also referred to as CAT (computerized axial tomography), scans of the brain show soft tissue and are often taken soon after the injury as a way to identify swelling, bruising and bleeding. Occasionally MRI (magnetic resonance imaging) scans, which also show soft tissue, may be done to look for subtle damage. An MRI scan may be utilized following a period of recovery as it is helpful in detailing the intricate areas of the brain.
After the initial evaluation, a physician may recommend that an ICP (intracranial pressure monitor) be placed. The monitor, which is inserted through a small opening in the skull, allows for continuous monitoring of the pressure in the brain. Treatment decisions are frequently based on the ICP measurements.
Occasionally, it is necessary to measure the electrical activity in the brain with an EEG (electrical encephalograph). Wires are attached to the scalp in several locations and brain waves are monitored to look for abnormal activity.

As Fraturas cranianas, ou quebra dos ossos que rodeiam o cérebro, podem ocorrer com ou sem dano na área do cérebro que está por baixo da fratura. Com o passar do tempo o osso sara. Podem ser necessários tratamentos adicionais se houver lesão no cérebro por baixo da fratura.
A Concussão é a forma mais ligeira de traumatismo cerebral. Quando alguém tem uma concussão pode desmaiar ou perder a consciência por um curto período de tempo, ou sofrer uma breve perda de memória ou de outras funções do cérebro. Geralmente, não é necessário tratamento, no entanto, é importante vigiar o paciente de perto para verificar se há sinais de que uma lesão mais grave possa ter ocorrido.
Numa contusão cerebral, uma parte do cérebro é magoada e surgem pequenas áreas de hemorragia e inchaço que podem ser vistas nos exames. Os pacientes com contusões queixam-se frequentemente de dor de cabeça, náusea e pensamento lento. Os pacientes com contusões devem ser vigiados de perto para se verificar se existem sinais de inchaço adicional, hemorragia ou outras lesões do cérebro.
Por vezes surgem lesões em lados opostos do cérebro, quando durante o impacto o cérebro bate num lado do crânio e depois ressalta contra o lado oposto (Figura 3).
Pode ocorrer uma lesão generalizada do cérebro quando forças de tensão ou de resistência são aplicadas em todo o cérebro. Como consequência podem ocorrer danos nas conexões, ou nos axónios, na parte mais profunda do cérebro, denominados de lesão axonal difusa. Em resposta, pode ocorrer um inchaço em todo o cérebro. Uma vez que os axónios não podem ser reparados através de uma intervenção médica, o tratamento é direcionado para a prevenção de danos adicionais. Uma hemorragia no cérebro ou ao seu redor provoca muitos danos e geralmente é tratada por via cirúrgica.
Dá-se o nome de hematoma epidural (figura 4) à hemorragia que ocorre entre o crânio e a dura, a fina camada que envolve o cérebro. Se o coágulo for grande ou estiver a aumentar de tamanho é realizada uma cirurgia para remover o coágulo e retirar a pressão do cérebro.
A hemorragia também pode ocorrer entre a dura e o cérebro. O coágulo forma o que se dá o nome de hematoma subdural (Figura 5). O hematoma subdural agudo ocorre na altura da lesão e está muitas vezes associado a danos na área do cérebro que está por baixo do hematoma e a inchaço intenso.
O hematoma subdural crónico ocorre mais frequentemente nos idosos a seguir a uma lesão relativamente ligeira na cabeça. Em alguns casos, uma pequena quantidade de sangue no espaço subdural vai causar um aumento do espaço ao longo do tempo, exercendo uma pressão crescente no cérebro. Em ambos os casos, muitas vezes é necessária uma cirurgia.
Uma hemorragia intracerebral é um coágulo dentro do cérebro (Figura 6). Pode ser necessário uma cirurgia para remover o coágulo, mas a cirurgia tem riscos significativos, porque para remover o coágulo pode ser preciso passar através de áreas que estão envolvidas no funcionamento normal do cérebro.
Por vezes, ocorre uma hemorragia nos espaços preenchidos com LCR. O sangue pode bloquear o fluxo do LCR o que faz com que pressão nos ventrículos aumente, e com que estes se expandam. A esta condição dá-se o nome de hidrocefalia (Figura 7). A hidrocefalia é geralmente tratada com um tubo de drenagem ou ventriculostomia, que drena o fluido para um saco de drenagem fora do corpo ou para debaixo da pele para dentro da barriga.
Qualquer lesão no cérebro pode afetar a pressão intracraniana (PIC), isto é, a pressão dentro do crânio. Como o crânio é rígido e não pode esticar, os inchaços, os coágulos sanguíneos ou o bloqueio do fluxo de LCR podem fazer com que o que tudo o que está no interior do crânio fique apertado e com que a pressão dentro do cérebro aumente. Grande parte dos tratamentos do traumatismo crânio-encefálico tem como como objetivo manter a pressão intracraniana o mais perto do normal possível.
A pressão de perfusão cerebral é a pressão que empurra o sangue para o cérebro e que fornece oxigénio ao cérebro. Se a pressão sanguínea for baixa ou a pressão intracraniana for elevada, ou ambas, então a perfusão cerebral será baixa, e a circulação sanguínea para o cérebro diminuirá. Isto pode provocar um nível baixo de oxigénio e eventuais danos cerebrais (Figura 8).
Os pacientes com traumatismo craniano são muitas vezes submetidos a testes exaustivos para determinar a dimensão e a localização do dano.
Um profissional de saúde, realiza um exame físico por um profissional de saúde que inclui, especificamente, um exame neurológico completo que avalia as funções cerebrais, tais como:
• força;
• sensação;
• fala;
• visão;
• nível de consciência.
Os Raios X, mostram apenas estruturas ósseas e são usados para procurar fraturas nos ossos. A TC (tomografia computorizada), também denominada de TAC (tomografia axial computorizada), mostra os tecidos moles do cérebro e é normalmente realizada logo a seguir à lesão como forma de identificar inchaços, áreas magoadas e hemorragias. Por vezes também podem ser realizadas RM (ressonâncias magnéticas), as quais também mostram os tecidos moles, e podem ser realizadas para procurar danos mais subtis. A RM pode ser utilizada a seguir a um período de recuperação porque pode ajudar a pormenorizar as áreas mais complexas do cérebro.
Após a avaliação inicial, o médico pode recomendar que seja colocado um monitor da PIC (monitor de pressão intracraniana). O monitor, que é inserido através de uma pequena abertura no crânio, permite uma monitorização contínua da pressão no cérebro. As decisões relativas ao tratamento são, muitas vezes, baseadas nas medidas de PIC.
Por vezes, é necessário medir a atividade elétrica do cérebro com um EEG (eletroencefalografia). Os sensores são colocados em vários sítios no couro cabeludo ligados por cabos e as ondas cerebrais são monitorizadas para procurar atividade anormal.
