This is a phase 3, multicenter (multi-national), open label, parallel group, randomized, active-controlled study in approximately 150 patients with documented carbapenem-resistant Gram-negative bacterial infections. Patients meeting eligibility criteria and assessed by the investigator to require 7-14 days of intravenous treatment in hospital will be randomized (2:1) to either S-649266 2 g administered intravenously over 3 hours, every 8 hours (q8h) or BAT.
Study Population:
Patients with the following infections caused by a carbapenem-resistant Gram-negative pathogen will be enrolled:
• HAP/VAP/HCAP or
• cUTI or
• BSI/sepsis
General Inclusion Criteria:
Patients who fulfill the following criteria at Screening will be included in the study:
1. Hospitalized male and female patients, 18 years or older at the time of signing informed consent
2. Patients who have provided written informed consent or their informed consent has been provided by legal guardian (Note: Country specific rules and local Ethics Committee approval for legal guardian informed consent will determine whether or not and how a patient unable to comprehend or sign the informed consent is allowed to be enrolled in the study)
3. Patients with clinically documented infection (HAP/VAP/HCAP, cUTI, or BSI/sepsis) caused by a Gram-negative pathogen with evidence of carbapenem resistance
4. Patients who have been treated previously with an empiric antibiotic other than the study medications, but failed treatment, both clinically and microbiologically, are eligible for the study if they have an identified pathogen which is non-susceptible to the empiric treatment and is a carbapenem-resistant Gram-negative pathogen before entering this study.

Pacientes que presenten consentimiento informado por escrito o pacientes cuyo tutor legal presente dicho consentimiento. (Nota: Si un paciente se encuentra inhabilitado para comprender o firmar el consentimiento informado, su participación en el estudio será determinada por las reglas específicas de cada país y la aprobación del comité de ética local para la presentación de consentimientos informados por parte de tutores legales.
Pacientes con infecciones documentadas clínicamente (NAC/ NAV/ NIC, infecciones urinarias o sanguíneas/septicemia) a causa de un microorganismo patógeno gramnegativo que presente resistencia a carbapenémicos.
