Since it is a copy of the original certificate, "...a extraí " in the context means that that clerk who made the copy " extracted ", or " retrieved " from the original document information to (re)-create the copy;
[ Extrai ] is present tense of doing said action while [ Extraí ] means the clerk did said action in the past or at that particular moment in time when the requestor asked for said copy of original document.

It looks as though there is an error in the text then because it definitely reads as 'a extraí'. Should it read 'a extrair' then perhaps? What follows is 'o referido e verdade e dou fé'

A extraí não faz sentido nenhum, já que extraí é o pretérito perfeito do verbo (eu extraí) A extrai, sem acento, já faz certo sentido, mas seria bom saber a que substantivo esse a se refere.
