In the certificate the clerk put a stamp with his name + 3° substituto do oficial.
Does it mean he is the officer's substitute here? Or is he a deputy clerk/ registrar?

You assumed correctly. The registrars have deputies, who replace him in occasional absences. Sometimes, more than one, as it seems to be the case here, where the person is the third deputy registrar.

this is what I use, because sometimes the same document will have other titles and I might need "deputy" for someone else. I believe but am not positive that these registrars have pre-determined alternates. To me, "interim" or "acting" is for "em exercicio" when the registrar might be absent for a longer time or a new one hasn't been appointed yet.
