Have the local exchanger fabricator extract the tube in question and install pocket tubes in its place. A pocket tube is a little longer than the thickness of your tubesheet and open only on one end. After installation they are rolled in place. Once you have the tube you can cut it up and split it to evaluate the failure mode.

The threaded version with connection to a ½" thread is intended for clean media. At a media temperature higher than +80 °C, a water pocket tube is used to prevent too high a temperature at the sensor.

Max. ambient temperature
–20 to 70 °C Max. medium temperature 70 °C, higher medium temperatures are possible if the above limiting values at the switching device are not exceeded by suitable measures (e. g. water pocket tube). At ambient temperatures below 0 °C, ensure that no water condensation can arise in the sensor and in the switching device.

If Marcia could confirm if 'her' water pocket tube is part of a transmitter for pressure and level or a pressure monitor, it would help.
