Mas como cota é a posição numa escala, e os dados que se seguem são de posicionamento num arquivo, penso que o significado literal será talvez possível.

COTA is a word used for inventoriable items, such as "lot". Tabela is a list, or Table. The rest means: Act No, Letter No., which is pretty consistent with a b-certificate. Tabela: could it be Tabeliao? Common sense says, do a good job translatg the b-certificate itself, and for this write: "Illegible" if it's illegible, or translate it literally if it is legible. The first word probably refers to the registration entry number, second word is definitely table, the third one (Ato) does not exist in the portuguese language maybe its an abbreviation (?).

I have come across something similar. Since one definition of "cota" is assessment and there is charge of R$ 1.76, I am assuming this is a "fee" listed on Schedule II (in my case) "COTA TABLA II". "Ato" can be "instrument", to use something a bit imprecise.
