Glossary entry (derived from question below)
Dutch term or phrase:
V-cxfl Toe
English translation:
occlusion SVG-LCX ; occlusion of saphenous Vein graft to left circumflex (cflx) coronary atery
Dutch term
V-cxfl
Earlier in the text, the past medical history states "CABG LIMA-LAD, CABG RIMA-RAC, CABG SVG-LCX".
I suspect "Toe" may be short for "transoesofageale echografie" (transesophageal ultrasound), though the official acronym would be TEE, not TOE.
It is a diabetic patient with a protracted cardiovascular history, who finally underwent toe amputation, followed that same year by lower-leg amputation. (Your typical 'Salami protocol', as it is sometimes labeled rather unrespectfully in Dutch.)
Any ideas?

Proposed translations
occlusion SVG-LCX
venous graft to circumflex coronary artery, given PMH SVG-LCX
--> Saphenous Vein Graft to the LCX (circumflex coronary
artery)
Evaluation: LIMA-LAD is OK, RIMA-RAC is OK, V-cxfl is occluded.

Discussion

Waar staat precies:
LIMA-LAD OK, RIMA-RAC OK, V-cxfl Toe
LIMA-LAD OK, RIMA-RAC OK, V-cxfl Toe
artery)
V-cxfl --> waarschijnlijk veneuze graft naar de circumflexus
